Transformations Care Network is excited to announce the following changes to our employee benefits program effective January 1, 2025:

• New medical carrier will be Blue Benefit Administrators of Massachusetts

• Two medical plans will now be offered - a traditional PPO plan and a Health Savings Account qualified High Deductible Health Plan

•  Prime Therapeutics will be our new pharmacy benefits manager

•  Guardian replaces Sun Life for all other coverages and our vision network will remain with VSP

• A Dependent Care Flexible Spending Account and a Health Savings Account will now be available through Navia
